This happens when Cyrus Drake tells you, (This is directly from the dialogue) 'Take this Knowledge Crystal. Find her tomb and use the crystal there, it will store her spirit within.' 'Then you must return the crystal to me, and I will take the necessary next steps.'
Once you go to her tomb, it tells you to 'Use the Crystal on the tomb' not, 'Use the Crystal in the tomb.'
You have to go inside and fight your way in to get to her tomb. The door isn't her tomb, it's just the entrance to her tomb. Not a typo, just a miscomprehension on your part.
